Step 1 – Submit documents
- New Student Information Form (available at the front office)
- Birth Certificate – Copy
- Immunization Record – Copy
- Additional documentation is required if the student has received Speech, Occupational, or Physical therapy, has an IEP, has a 504 plan, takes medication at school, or has a condition that could cause an emergency
Step 2 – Complete Online Registration Form
- Complete once for each student (if you have 3 students, you complete it 3 times) using the code(s) you received at the assessment
Step 3 – Schedule and Complete Assessment
- Assessments will be held in May and June
- Schedule an assessment for each new student – contact Elementary Office for details
